Earlier this week, the POTUS incorrectly described a photo he held up as being proof of White Farmers that were killed in South Africa.

From what I can see the WH has not relased a statement on it but the author of the article from which the photo was taken did say the following "The White House did not respond to a request for comment. Andrea Widburg, managing editor at American Thinker and the author of the post in question, wrote in reply to a Reuters query that Trump had "misidentified the image.""

So either he unknowingly misidentified information he was given or he willingly chose to misidentify it. If it is the first scenario then that is not acceptible for someone that is on his staff to essentially set him up even if it was an honest mistake. The second scenario, is in line with how loose and fast he plays with the truth when speaking. Not much else to say.
