It only has to affect one. Only one pilot is actually flying, and at 1600 feet it needs to be recognized quickly. Spatial disorientation isn’t slow. It can affect you almost as soon as you enter clouds.

Then again those things were likely designed in the 70s, when engineers still occasionally had undue faith in scheduled maintenance and didn't think that every piece of technology was trying to actively kill or stymie them.

This was most likely skipped maintenance trying to save a buck, and pencil whipped inspections figuring the CVR recordings wouldn't be needed, and if they were, "oops".
