Accidents happen, and incidents with hardware as advanced as an F-35 are, on a long enough time line, unavoidable. But in this case, it seems that the real culprit is lax unit culture and a failure of oversight from leadership. This wasn’t the result of one person making an error. I’d bet the farm that this investigation probably uncovered some other BIG issues within that particular unit. If they’re able to overlook basic maintenance policy and procedure with respect to the handling and storage of HazMat, they are letting other things slide, too.
I am just relieved that this broader and potentially deadly unit culture failure was able to be sussed out and addressed before someone lost their life or was seriously injured. |
