Why can't it be both? Not saying he's schizophrenic but schizophrenic's think people are reading their thoughts I forgot the exact terminology for it, I could call a friend and get it or a simple google search would do but it's a common delusion. However, even with said delusion, the event could have still happened. Stranger things definitely have.

Let's back up a couple steps and look at this from a perspective loaded with a couple assumptions in as well.
Let's assume that:
A.) There really are extra-terrestrials who have some interest in Earth or mankind, and let's forget for a moment what 'form' these E.T.'s might take. And, ...
B.) Let's assume these E.T.'s are smart enough to realize the social / societal impact of the discovery of such an entity.
The logical conclusion we could draw from this would be that they would "enlist" (through whatever method) a group of people who were trustworthy enough to protect proof of their existence until such time as "humans" could properly (and maturely) deal with this entity.
This makes perfect sense to me, that such a group, call them MIB's, would exist. They could be anything, but the highest risk would be to make them non-human. The better alternative would be a " flashy thing" to erase their previous memories and make them oblivious to emotions and their terrestrial shortcomings.
In my mind, the 'facts is', any E.T. who has the ability to get here from some distant locale has the ability to transform mass based intelligence into mass-less knowledge. Maybe they also have the ability to 're-generate' this intelligence into a mass based object, or maybe they don't, but this doesn't change their much higher level of intelligence than humans.
Honestly, I would expect nothing less than some form of "MIB", sanctioned or unsanctioned by some government body.
On point, I think something like an 'agency' like this is much less likely today than it may have been back in say the 50's or 60's. The government today has become far more corrupt, and secrets equal power, and more importantly money (which equals power), so it's understandable why MIB's today are a rare thing (if they ever existed at all)
Equally, if I was an E.T., I would have seen the World culture as far more attractive in the 50's and 60's than I would view it today. There's too much hate and volatility today than there was then. The human race was a nicer place then. Now, it is a seething pool of hate and excrement with corrupted minds, influenced almost exclusively by propaganda. Today, if I was E.T....I'd just sit back and wait...wait for the world below to implode onto itself, to incinerate every footprint of its very existence.

As "mainstream" as UFOlogy is today and easily and openly talked about, one would think that if these MIBs were as described, there would be a TON of encounters and visits from them. I haven't heard of any in awhile. Have any of the well-known players and charlatans out there even mentioned contact with them?

Apologies for the long read but there's some pretty freaky pilot 'intimidation' described below following a daylight, close range, radar/visual, EM effect case from Mexico on May 3rd, 1975.

This is a report of UFO that have a behaviour which is suggesting intelligent control.
This is a case of physical effects on an aircraft.
This is a radar/visual case, where a visual sighting is confirmed by independent people detecting the presence of the objects on a detection device.
This is a case where the UFOs fly away in the direction of Mount Popocatepetl, obviously a hotbed of UFO reports and allegedly some sort of hideout for extraterrestrial aircraft.
This is a case where a pilot reports his encounter publicly.
This is a case where the witness has been medically checked and interviewed. If Hynek could not speak to Los Santos Montiel, at least Jerome Clark and others could.
This is a case where the witness stood firm with his account over time.
This is a case where the witness reports that he has been repeatedly intimidated by unidentified people probably from a foreign country (UFO folklore call them "Men in Black").

Speaking quickly in Spanish, with a strange "mechanical" tone, the man said "Look boy, if you value your life and your family's too, don't talk any more about this sighting of yours." Stunned speechless, Carlos turned his car around and drove home; he never made it to the television studio that day.
Carlos described his experience to Pedro Ferriz two days later who (interestingly) said that he had heard similar stories from UFO witnesses before..
And one of them also popped up later that year when the witness tried to meet Project Bluebook scientist Dr Hynek.

Do you work for Mexicana Airlines? Get out of here - and don't come back."
Carlos did as he was told. This proved to be Carlos's last encounter with the strangers. Two years later, when talking to UFO researcher Jerome Clark and his friend Richard Heiden, he recalled "They were strange. They were huge, taller than Mexicans are, and they were so white. But the strangest thing of all is that all the while they were in my presence I never saw them blink."

Well they're certainly mysterious mate (whatever their origin) but think the CIA has got media manipulation pretty much covered these days.
Lots of different 'terrestrial' MIB opinions out there and the ones I've read range from the NSA, NRO, DIA, the Department Of Energy and Secret Service right through to Britain's DSTI, DI55 and P&SS - some of the reported MIB characteristics and behaviours do sound extremely strange though.
The Defense Department and DIA do pop up a lot in UFO research (and many UFO documents contain their routing and transmittal codes) so many folks have speculated they're heavily involved with the subject ...but when it comes to pale, emaciated, tall, bald, long fingered, lipstick wearing men not knowing what jello, spoons or pens are then I'm completely flummoxed.
Below is some info on the infamous MIB Jello case (or jelly as we call it in England).

He then stared at the spoon Mrs. Butler had given him, as if he had no comprehension of what it was. He then awkwardly picked up the bowl and proceeded to try and drink the Jello. Mrs. Butler went silent and stared in disbelief. Major French did not hang around, realizing, it seems, that Mrs. Butler knew something was not quite normal – which is an understatement of absolutely epic proportions.
