My Doctor Visit summery! Wow!

On 12-08-2025 I went to my for my 6 month Dr. visit. I had a heart replacement in 2023, I was born with a bad heart heart valve. I get a checkup every six months. 

The next morning they email me a summary of my visit! The summary is all done by AI! Everything was super wrong. I am adding a screenshot below. I have never been to Vegas! I could not post the meds they added because they were not even my meds plus it had my Dr’s name listed and another patient. They ended up billing my insurance using AI for another person. I am only on two meds for my heart. AI put info into my pharmacy for refills of 7 meds plus my meds. I ended up calling Mr Doctors office after CVS text me that my RX’s are ready. That is when the person on the phone said but we already billed your insurance! Lol I said so fix this big big mess!

What will the record show

What would a "journalist report?"  Should it come down to that....
