There are a lot of reasons for clots in the living -- but a mortician can't tell if a clot was present when the person was alive (and was not a cause of death) or if it happened as part of the death process (that was mentioned in one of the links I provided, I think.)

And the only way you could tell the difference is through a microscope (mentioned in that same article.)

Pathologists would use microscope on samples.  Morticians don't do this.  Morticians wouldn't have a cause of death and a list of "other conditions" -- the pathologist would.  Morticians wouldn't know if the patient had one of these problems (https://my.clevelandclinic.org/health/di...ble-states)

Also, what's the source for "worldwide" claims?  Speaking from experience (I was in medical school as a teaching assistant), weekly journal reviews that are part of the usual process in training physicians (students sit down and read through a lot of research and present "here's something wild and unusual") catch weird things like this.  I was the one in my journal club that caught the two papers published almost simultaneously about Kaposi's sarcoma in gay men -- the start of the identification of the AIDS epidemic.

It's not always a paper; sometimes it's in the form of letters of comment.

So... where are the published reports by pathologists and physicians about this?  There should be a lot of this, and it should be global and impossible to cover up.
